PROSPECTIVE DONORS
Step 1: Donate
Create your account and apply to be an embryo donor.
Step 2: Submit Consent Forms
Part A: Before proceeding further, the "Informed
Consent for Receipt of Donated Embryos / Waiver of Liability" must be signed, notarized and the original returned to the NEDC. Click here to download this form.
Part B: If you wish to have an open relationship with the receiving couple, the "Open Donation Agreement" must be completed, signed and returned to the NEDC. It does not need to be notarized at this time as the final document will be drafted at the time a match is completed. Click here to download this form.
Step 3: Submit Medical Information
Part A: Contact the center where your IVF procedure was performed and request a "Release of Medical Record" form so you can arrange to have the results of the blood testing for sexualy transmitted diseases (STDs) performed prior to the IVF procedure where the embryos were cryopreserved sent to the NEDC.
Part B: Submit results of current blood tests for STDs to the NEDC. Physician signed Laboratory Request Forms with required tests marked will be mailed to you after you complete Step 1.
Step 4 Moving Embryos
Part A: Contact the center where your frozen embryos are stored and complete the necessary consents they require in order for your embryos to be transported to the NEDC.
Part B: Let the NEDC know you have completed Part A above and inform the NEDC of the contact information of the center where the embryos are stored.
Part C: The NEDC then contacts the center where the embryos are stored and arranges to send our shipper tank to pick up the embryos at our expense.
Step 5: View Potential Adoptive Families
Once the embryos have been moved to the NEDC, you will be provided with a password via email to view potential adoptive families to whom you might consider donating your embryos.
