PROSPECTIVE RECIPIENTS

Step 1: Apply and Pay your Application Processing Fee

To apply to be an embryo recipient, create your account and apply to be an embryo recipient.

Step 2: Complete the Followup Packet

Once one of our representatives has confirmed acceptance ofyour application, the NEDC will send you a followup packet.

Part A: Submit your information that Potential Donors can view
online. Be prepared with your Recipient ID Number from Step 1
above and use the form in the right column to submit your information.

Step 3: Forms and Medical Information

SUBMIT CONSENT FORMS

Part A: Before proceeding further, the "Informed Consent for Receipt of Donated Embryos / Waiver of Liability" must be signed, notarized and the original returned to the NEDC. Click here to download this form.

Part B: If you wish to have an open relationship with the donating couple, the "Open Donation Agreement" must be completed, signed and returned to the NEDC. It does not need to be notarized at this time as the final document will be drafted at the time a match is completed. Click here to download this form.

SUBMIT MEDICAL INFORMATION

Part A: Submit results of blood test for sexually transmitted diseases (STDs) to the NEDC. Physician signed Laboratory Request Forms with required tests marked will be included in the followup packet mailed to you upon approval of your application under Step 1.

Part B: Medical Records from any previous infertility assessments performed on both partners of the applying couple should be submitted to Dr. Jeffrey Keenan at the NEDC.

Step 4: Home Study

Once everything from STEP 1 - STEP 3 is completed, received and approved by the NEDC, the NEDC contacts Bethany Services to start your home study. A social worker in your area will contact you to start the process. If you already have an approved home study and it is current, the NEDC will need to receive a copy of it for review. In most cases home studies from other agencies meet the NEDC requirements.

Step 5: Initial Medical Consultation Appointment

Once the home study is underway or your current home study is approved, a nurse from the NEDC will call to schedule your initial appointment at the NEDC.

Step 6: View Potential Embryo Donors

After completing steps 1 through 5 and being approved to receive embryos, the embryo matching process begins. For those couples pursuing Open Donation, the Donor couple first selects the receiving couple and the receiving couple must then decide to accept the donor couple. For an anonymous (closed) adoption, a list of available embryos will be given after the initial appointment. You may also receive a special login via email that will allow you to view donated embryos that you might consider adopting.

Step 7: Embryo Transfer

Once donor is in place, the embryo transfer is scheduled.
